What Is a Quality Management System?

A Quality Management System (QMS) is the set of documented processes, procedures, responsibilities, and policies that guide an organisation in achieving its quality objectives. ISO 9001:2015 defines the requirements a QMS must meet to be certified. The standard is structured around the Plan-Do-Check-Act (PDCA) cycle and seven quality management principles: customer focus, leadership, engagement of people, process approach, improvement, evidence-based decision making, and relationship management.

Key requirements under ISO 9001:2015 include:

  • Context of the organisation: Understanding internal and external issues, and the needs of interested parties.
  • Risk-based thinking: Identifying risks and opportunities that could affect your ability to meet customer requirements.
  • Documented information: Maintaining and retaining evidence that processes are controlled and objectives are being met.
  • Performance evaluation: Monitoring customer satisfaction, conducting internal audits, and reviewing management system performance.
  • Continual improvement: Addressing nonconformities and driving systemic improvements through corrective action.

The Certification Process: Stage 1 and Stage 2

ISO 9001 certification is carried out by an independent certification body (CB) such as BCERT. The process involves two mandatory audit stages:

Stage 1 — Documentation Review: The auditor reviews your QMS documentation to confirm that your system meets the standard's requirements in principle. This includes your quality manual (if you have one), scope statement, quality policy, objectives, risk register, and key procedures. The Stage 1 audit also scopes the Stage 2 audit and identifies any areas that need to be addressed before proceeding. It is typically conducted remotely.

Stage 2 — Implementation Audit: This is the main certification audit, conducted on-site. Auditors verify that your QMS is fully implemented and operating effectively across the scope. They will interview staff, review records, observe processes, and assess whether the system produces the intended outcomes. If only minor nonconformities are found, the CB can recommend certification subject to corrective actions. Major nonconformities require resolution before a certificate is issued.

Surveillance Audits and Recertification

ISO 9001 certificates are valid for three years, but the certification cycle does not end at Stage 2. To maintain certification, your organisation must undergo:

  • Surveillance Audit 1 (Year 1): A focused audit, typically covering internal audit results, customer complaints, corrective actions, and a selection of key processes.
  • Surveillance Audit 2 (Year 2): A broader surveillance covering additional areas of the QMS.
  • Recertification Audit (Year 3): A full audit of the QMS similar in scope to the original Stage 2. Successful completion re-issues the certificate for another three-year cycle.

Failing to undergo surveillance audits on schedule can result in suspension or withdrawal of certification.
